Antibiotic Allergy
An antibiotic allergy is an immune system reaction to certain medications within the antibiotic group. It can manifest as a rash, itching, swelling, or even more severe symptoms. This reaction differs from non-immune-related side effects and can occur either during treatment or some time after completing the course.
Causes of Antibiotic Allergy
It is important to understand that not all adverse reactions to antibiotics are allergies. People often confuse allergic symptoms with toxic or side effects related to the medication’s general impact on the body.
An allergic reaction occurs when the immune system perceives an antibiotic as a “foreign” substance and activates defense mechanisms against it. The body produces IgE antibodies or triggers other immune pathways, leading to the release of inflammatory mediators like histamine. This is what causes allergy symptoms.
- While penicillin and sulfonamides are the most common triggers, any antibiotic can cause a reaction in sensitive individuals.
- A predisposition to allergies may be genetic, and the risk increases if a person has existing allergic diseases or multiple sensitivities.
How the Allergic Reaction Develops
The process involves the immune system interacting with the antibiotic molecule or its metabolites. During the first contact, IgE antibodies bind to the antibiotic and “remember” it as an allergen.
Upon repeated exposure, these antibodies activate mast cells and basophils, triggering the release of histamine. This causes blood vessel dilation, tissue swelling, and typical symptoms such as rash and itching.
The reaction can affect the skin, respiratory tract, mucous membranes, and internal organs. In severe cases, a systemic reaction may occur, requiring emergency medical care.
Diagnosis of Antibiotic Allergy
Diagnosis begins with a detailed medical history. The doctor identifies which drugs were taken, when the symptoms appeared, and whether similar reactions occurred in the past. This helps distinguish an actual allergy from other side effects.
To confirm the allergy, doctors may use skin prick tests with small doses of potential allergens or specific IgE blood tests. In some cases, a monitored drug provocation test may be conducted for a definitive diagnosis.
Treatment of Antibiotic Allergy
- The first and most critical step is to stop taking the medication immediately. To alleviate symptoms, antihistamines are used for itching and rashes, while corticosteroids may be prescribed for more severe reactions.
- In the event of an anaphylactic reaction, emergency epinephrine (adrenaline) is required, along with respiratory and circulatory support in a hospital setting.
- Once the acute symptoms are managed, the physician can select an alternative antibiotic from a different class that does not have cross-reactivity.
Prevention of Allergic Reactions
To minimize risk, never take antibiotics without a prescription. Always inform your doctor about any known drug allergies before starting a new treatment.
If an allergy to a specific group is confirmed, it must be noted in your medical records, and medications with a similar molecular structure should be avoided. If necessary, you can consult an allergist in the future to undergo testing and identify safe alternatives.
